8th Annual EPHMA Golf Tournament

If you have not already signed up to be a sponsor or golfer in this year's EPHMA Golf Tournament, we need your help. The tournament will be on Friday November 4th. Just click the link below to download the signup form.
Proceeds from the tournament are used for the HMA’s Scholarship fund and the First Tee Program.
We need hole sponsors ($125.), individual golfers ($125), team of 4 ($400.), team and hole sponsor ($500.), beverage donations, items for the raffle (gift baskets, gift certificates, etc) and items for the goodie bags to be given to each golfer. Hole sponsors need to submit your name ASAP so we can have the sign printed. If you need someone to pick up donations, please let me know.
